• Comprehensive Psychiatric Evaluation – $325
  Duration: 60–75 minutes
  A full diagnostic assessment exploring your mental health history, symptoms, lifestyle, and goals. Includes discussion of treatment options (medication, therapy, and integrative supports). This session lays the foundation for your care plan.

• Follow-Up Visit – $175
  Duration: 25–30 minutes
  Ongoing medication management and brief therapy check-ins. Focused on progress, side effects, symptom tracking, and adjustments as needed.

• Extended Follow-Up – $350–400
  Duration: 50–60 minutes
  For complex cases, medication changes, or when additional therapy time is beneficial. Often used for trauma work, multiple diagnoses, or collaborative family sessions.

• Therapy-Integrated Follow-Up (Medication + Psychotherapy) - $325–$400
  Duration: 45–60 minutes
  Combines medication management with supportive psychotherapy using CBT, DBT, and trauma-informed approaches.

• Administrative or Form Completion Services – $50–$150
  Covers non-clinical administrative requests such as FMLA, disability, or school/employment paperwork. These are not included in visit fees or insurance billing.

  • To ensure that all paperwork accurately reflects your clinical status and treatment history, form completion requires an established provider–patient relationship. This policy helps maintain ethical and accurate documentation consistent with state, federal, and insurance guidelines.

  • Fees are based on the time and complexity of the request and must be paid prior to form release.
    Turnaround time is typically 7–10 business days from receipt of both payment and all required information.
    Expedited requests (under 5 business days) may incur an additional charge when scheduling allows.

Returning Patients

If you’ve seen me within the last three years, you’re considered an established patient—but because I’ve transitioned to my own practice, I need all returning patients to complete a one-time intake setup in my new system.

When you schedule your first visit at Bluebird Psychiatric Care, please choose the New Patient Intake appointment option. This does not change your clinical or billing status. It simply triggers the legally required onboarding forms for:

• Consent to treat

• Telehealth consent

• Practice policies

• HIPAA privacy forms

• Updated contact and emergency information

• Payment authorization

Once this packet is completed, all future visits can be scheduled as regular follow-ups.
